Fluphenazine blocks postsynap- tic mesolimbic dopaminergic D1 and D2 receptors in the brain, depresses the release of hypothalamic and hypophyseal hormones and it is also known to depress the reticular activating system hence affecting basal metabolism, body tempera- ture, wakefulness, vasomotor tone, and emesis. Nortriptyline increases the synaptic concentration of norepinephrine and serotonin in the central nervous system [CNS]. Unlike amitriptyline, nortriptyline has greater effect on re-uptake of norepinephrine than that of serotonin.
Concurrent administration with CNS depressants; patients with glaucoma, prostrate enlargement, cardiac failure and cardiac infarction; the drug may impair alertness and ability to drive a car or operate machinery.
History of gradual epilepsy or organic brain damage; blood dyscrasias; severe cardiac insufficiency; renal or hepatic damage; patients on MAO inhibitors; younger children, and hypersensitivity to any component of the formulation.
Tardive dyskinesias (mainly high doses administered for a long period); rarely, malignant neuroleptic syndrome and sudden death can occur; nortriptyline component can cause agranulocytosis (though rarely).
The safety in human pregnancy has not yet been established. The drug is not recommended for use in lactation as it is excreted in breast-milk.
