Mode of Action
It is an antagonist of N-methyl-D-aspartate [NMDA] receptors; affects muscarinic receptors, opioid receptors and voltage-sensitive calcium channels in spinal cord and brain, and descending monoaminergic pain pathways.
Elevated intracranial and intraocular pressures are worsened; psychiatric disorders can worsen; in serious eye injuries intraocular content can be lost; as ketamine raises blood pressure and heart rate it can worsen chronic heart failure, tachyarrhythmias and hyperthyroidism.
Disease conditions that can be worsened by elevated blood pressure and /or heart rate
Increased blood pressure, pulse rate, intracranial pressure, intraocular pressure, salivation, lacrimation and muscle tone; emergency reaction that involves disturbing dreams, restlessness, hallucination, irrational behaviour; postoperative nausea and vomiting; diplopia; nystagmus; erythema; hypotension and bradycardia [as opposed to the more frequent raised BP and increased purse rate]
Induction: IV:1-4.5mg/kg administered within a period of 60 secs, onset of action is 30 secs with a duration of action of 5-10 mins. IM:6.5 -13mg/kg (usually 10mg) with onset of action of 3-4 minutes a duration of action of 12-25 mins. IV infusion 0.5 - 2mg initially then 10-45mcg/kg/min.
